Campaign Summary = WE ARE PUSHING HARDCORE THIS CAMPAIGN THIS MEDICAL ALERT CAMPAIGN AND PAY $3.00 PER “WARM TRANSFER” WITH ONLY 10% SCRUB MAXIMUM.* *THE AGENT MUST READ THE SCRIPT VERBATIM TO GET THIS… RECORDINGS WILL BE LISTENED TO AND IF THERE IS NON-VERBATIM/ANY DEVIATION THEN IT WILL BE A NON-PAYABLE “WARM TRANSFER”.

Forecasted length of campaign (Minimum 12-Months) = 12 Months+

Projected campaign start date = Immediate

Product or service focus = Medical Alert

Service Level Agreement = We're looking for 1-200 Transfers per day (number of Transfers may increase after trial period). Can run as a stand-alone or jointly related add-on campaign,

performance metrics monitored daily.

Training = Script Provided

Set number of professionals needed to begin campaign = 5 Agents (Monday thru Sunday)

Payment Method = Wire Transfer / Western Union

Payment Schedule = Weekly – Net 14

How Will Sales Metric Be Captured = Reporting & Provided Tracking Spreadsheet

Do you wish to have the campaign metric monitored = Client will review reports and online tracking portal provided and share reports with call center.

Hourly Rate Proposal (USD) = N/A

Pay-out Rates = $3.00 Per transfer (Customer must remain on the line for 30 seconds after being transferred)

Conversion Rate = 90% of the customers remain on the call for 30 seconds after being transferred

In-Bound / Out-Bound / [Both] = Outbound Only

Technical Equipment Provided = Predictive dialer, the ability to transfer calls. All calls must be recorded in a WAV or Mp3 format.

Leads Provided = yes

Additional Guidelines of Campaign = 1. Customers must understand and confirm they want the alert device. 2. NO HARD SELLING, PERIOD! 3. Must be polite and courteous to customer. 4. Customers must have an active credit card, but do not ask for CC number. 5. Must follow script. 6. If calls are blindly or forced submitted as a lead, contract will be terminated! 7. AGAIN, NO HARD SELLING! 8. Cannot “speed read” script. 9. Treat the script and customer with integrity! 10. 100% of calls must be recorded
